Overview
Advance Care Planning (ACP) is an ongoing process of planning and preparing for future health and personal care. It includes conversations that communicate individuals’ personal beliefs, values, and healthcare preferences to people important to them, as well as to their healthcare team.
Having a quality ACP ensures that this process is person-centred, informed and guided. In the event that individual loses the capacity to make decisions or speak for yourself, the Advance Care Plan (ACP) will help your loved ones and healthcare team act in your best interests based on your prior expressed wishes.
The program offers comprehensive guidance on initiating and facilitating General ACP conversations.
General ACP are conducted for individual who is generally healthy and well.

Certification
Upon completion of the course, the participants will receive a e-certificate of achievement.

- 9 hours classroom training with assessment (role-play)
- 2 hours follow-up Zoom Virtual Session

- Pre-Course MCQs – Maximum of 2 attempts to obtain 80% passing scores
Continuing Professional Education (CPE) Points
