Overview
Advance Care Planning (ACP) in Singapore is a vital process that empowers individuals to make informed decisions about their future healthcare. ACP involves discussions about personal values, beliefs, and preferences for medical care, ensuring that one’s wishes are respected even when they are unable to communicate. The program offers comprehensive guidance on initiating ACP conversations, understanding legal implications, and documenting care preferences. We aim to support individuals and their families in making thoughtful and informed choices, fostering peace of mind and ensuring dignity in healthcare decisions.
Learning Objectives
Participants will learn about the background and significance of ACP in Singapore, develop proficiency in facilitating general ACP discussions, and gain expertise in effectively communicating and documenting patients’ healthcare preferences. By the end of the course, attendees will be equipped to ensure that patients’ values and wishes are respected, promoting patient-centred care and informed healthcare decision-making.

Certification
Upon completion of the course, the participants will receive a Certificate of Performance from HMI Institute in partnership with AIC.
Course Structure
The total course duration is 11 hours, comprising 9 hours of face-to-face training and an additional 2-hour Zoom session held 4 weeks later. All ACP facilitator trainees are required to participate in one ACP facilitation before attending the follow-up virtual session.
Participants must pass a prerequisite quiz with the following conditions:
Quiz Structure:
The quiz consists of 20 questions.
A passing score is 80%, which requires correctly answering at least 16 out of 20 questions.
Please note that the MCQ quiz must be completed in one sitting. Exiting the quiz before completion will result in your progress being lost and you will have to start over from the beginning. Ensure that you have sufficient time and a stable internet connection before starting the quiz.
Attempt Limit:
Participants are allowed a maximum of 2 attempts to pass the quiz.
If both attempts result in a failing score, participants will need to reschedule to the next available course date.
