Overview
Advance Care Planning (ACP) is an ongoing process of planning and preparing for future health and personal care. It includes conversations that communicate individuals’ personal beliefs, values, and healthcare preferences to people important to them, as well as to their healthcare team.
Having a quality ACP ensures that this process is person-centred, informed and guided. In the event that individual loses the capacity to make decisions or speak for yourself, the ACP will help your loved ones and healthcare team act in your best interests based on your prior expressed wishes.
This training offers comprehensive guidance on initiating and facilitating Preferred Plan of Care (PPC) ACP conversations.
PPC ACP are conducted for individual with progressive life-limiting illness, suffering frequent complication or for individual at their advanced stage of illness.

Learning Objectives
- Describe the value of advance care planning
- Demonstrate facilitation skills needed to hold a PPC conversation with patients who have serious illnesses

- Proficiency in English, ability to commit to the entire 11 hours of training
We encourage all participants to complete the General ACP Facilitator Training before attending the PPC ACP Facilitator Training. The General ACP Facilitator Training serves as a foundational training to enhance your understanding of Advance Care Planning (ACP) and improves communication and facilitation skill.
If you are keen to proceed with the PPC Facilitator Training without first completing the General ACP Facilitator Training, kindly note the following:
- ACP facilitators can only facilitate and document the types of ACP for which they are certified in.
- For funding to be accorded, learner(s) shall fulfil the pre-disbursement criteria on attendance and “Pass” in assessment.

Certification
Upon completion of the course, the participants will receive a e-certificate of achievement.
