Supervisory Training in Mini Quick Test Centre ART Swabbing Operations

Course Objectives

This module is approved by Health Promotion Board (HPB) an extension of the Supervisory Training in ART Self-Swab to equip corporates with the knowledge and skills on how to supervise ART self-swabs for staff and the public, according to the Ministry of Health guidelines and protocols.

Pre-Requisite

This course is for participants who have completed the Supervisory Training in ART Self-swab (Employer-led Supervised Self-Swab).

Certification

Participant will be issued with “Certificate of Attendance” upon fulfilling 100% attendance course requirements.

Participants and/or their employers will receive the electronic certificates within 5 working days via email*.

*Course fee is non-refundable in the event that the participant does not fulfil the course requirements.

Registration

Step 1: Company representative to complete Form 1 and Form 2; email completed forms to swab@hmi.com.sg (incomplete forms will cause a delay in registration)

Step 2: Administrator will provide company representative available training time slot for registration. *Registration should be made at least 10 working days prior course commencement date.

Step 3: Upon successful enrolment, company representative will receive an invoice for payment.

Step 4: Company to make full payment at least 7 working days before course commencement.

HMI Institute reserves the right to withdraw or re-schedule the class if the minimum class size is not met.

Registration for all HMI Institute’s training courses are to be done directly on our official website. Communications are done via email address domain @hmi.com.sg. Please note that HMI Institute is not affiliated to any agencies/ companies for any form of recruitment activities.

Fees Payment Schedule

Upon enrolment, invoice will be emailed to the company officer indicated in the application form.

Course fee must be paid before course commencement. Payment must be made at least 7 working days before course commencement.

Course fee is strictly non-refundable for no show (eg. absent from the course/ does not fulfil the course requirement and assessment).

For any replacement or changes, a written notice must be submitted and received by HMI Institute at least 5 working days before course commencement.

Mode of Payment

Payment can be made via this mode and quote the invoice number when making payment. Confirmation email will be sent to you upon successful payment and confirmation of the required course fee.
